Direct Primary Care (DPC)

Weight Management
Our comprehensive medical weight management program is designed to help you achieve sustainable, long-term results through a personalized and evidence-based approach. We focus on identifying and addressing the factors that influence weight, including nutrition, physical activity, metabolism, lifestyle habits, and underlying medical conditions. Treatment plans may include individualized nutrition recommendations, exercise guidance, behavioral support, laboratory evaluation, and, when appropriate, GLP-1 or GIP/GLP-1 medications. Our goal is to provide ongoing education, accountability, and medical support to help you improve your health, build lasting habits, and feel your best at every stage of your journey.
Hormone Therapy
Our hormone optimization program for both women and men is designed to help address symptoms associated with hormonal imbalance, including fatigue, weight gain, decreased energy, reduced libido, mood changes, brain fog, and sleep disturbances. Treatment begins with a comprehensive evaluation, including a detailed health assessment and laboratory testing when appropriate. Using an individualized, evidence-based approach, we develop personalized treatment plans that may include hormone replacement therapy and lifestyle recommendations to support overall wellness. Our goal is to safely optimize hormone levels, improve quality of life, and help patients feel their best through ongoing monitoring and provider-guided care.
Acute Care Visits
We offer convenient same-day virtual evaluations for common acute illnesses, including cold, flu, and COVID-like symptoms, sinus infections, sore throat, cough, congestion, pink eye, allergies, urinary symptoms, and more. Treatment recommendations, prescriptions, diagnostic testing, imaging, and referrals may be provided when clinically appropriate.
As a virtual practice, we do not perform in-office testing. If you have completed over-the-counter testing for COVID-19, influenza, RSV, strep throat, or other conditions, please have your results available during your visit so we can provide the most informed and effective care possible. If an in-person evaluation or higher level of care is needed, we will guide you to the appropriate healthcare setting.
Peptide Treatments and IV Wellness Infusions
Our Peptide and IV Therapy services are designed to support overall wellness, recovery, performance, and healthy aging through personalized, provider-guided treatment plans. Peptide therapy utilizes targeted amino acid compounds that may help support energy, recovery, body composition, metabolic health, cognitive function, sleep quality, and overall vitality. IV therapy delivers fluids, vitamins, minerals, and nutrients directly into the bloodstream for rapid absorption and may help support hydration, energy levels, immune function, recovery, and general wellness. At Coastal Collective Health & Wellness, every treatment begins with a comprehensive assessment of your health history, symptoms, and wellness goals to determine the most appropriate approach. All therapies are prescribed and monitored by a licensed healthcare provider to ensure a safe, individualized experience. For eligible patients, select IV hydration and wellness infusions may also be available in the comfort and convenience of your home.
Referrals
When specialized care is needed, we can help coordinate referrals to appropriate specialists and healthcare services. A consultation is required prior to submitting a referral request to allow our provider to review your medical history, assess your symptoms or concerns, and ensure the referral is clinically appropriate. This evaluation helps facilitate continuity of care, provides the specialist with relevant medical information, and supports the most efficient and effective treatment plan for your healthcare needs.

MSN, APRN, FNP-BC
Brittnee Spooner is a Board-Certified Family Nurse Practitioner and Co-Founder of Coastal Collective Health & Wellness. She has been dedicated to the healthcare field since 2013 and earned her Master of Science in Nursing from Chamberlain University.
Throughout her nursing career, Brittnee has gained diverse clinical experience in primary care settings, trauma/neuro, urology, and home health. Her passion for patient-centered healthcare led her to pursue advanced practice nursing, where she focuses on building meaningful relationships with her patients and providing individualized, evidence-based care.
Brittnee is passionate about empowering and educating patients to take an active role in their health through preventative care, primary care, hormone optimization, weight management, and overall wellness. She believes the best outcomes are achieved by thoughtfully integrating evidence-based Western medicine with holistic approaches that support the whole person. Outside of work, Brittnee enjoys spending time with her family, being outdoors, boating, gardening, traveling, and practicing Pilates.

Brooke Revels is a Board-Certified Family Nurse Practitioner and Co-Founder of Coastal Collective Health & Wellness. She has been dedicated to the healthcare field since 2005 and earned her Master of Science in Nursing from Chamberlain University.
Brooke has extensive experience in urology, primary care, women's health, orthopedics, plastic surgery, peptide therapy, IV wellness treatments, hormone optimization, and weight management. She is passionate about empowering and educating patients to take an active role in their health and wellness while providing personalized, relationship-centered care. She is committed to delivering compassionate, evidence-based care and developing individualized treatment plans tailored to each patient's unique goals and needs. Brooke believes in combining modern medicine with practical lifestyle strategies to support long-term health and vitality.
Outside of work, Brooke enjoys spending time with her husband and two daughters, being outdoors, relaxing at the beach, staying active, and making memories with family and friends.

Direct Primary Care Memberships — $150/month
Choose from a 6-month or 12-month membership designed to provide personalized, relationship-centered healthcare. The 6-month membership includes up to 7 visits, while the 12-month membership includes up to 15 visits. Membership benefits include a comprehensive health assessment, ongoing primary care, personalized treatment plans, preventative care, chronic disease management, and unlimited secure messaging with your provider. Members also receive 10% off weight loss medications, peptide therapies, and IV hydration services.
Acute Care Visits — $75
Same-day and convenient virtual visits for acute healthcare needs, including cold and flu symptoms, COVID-19 concerns, sinus infections, sore throat, urinary symptoms, allergies, pink eye, minor skin conditions, and other non-emergency medical concerns. Our goal is to provide timely evaluation, treatment recommendations, and appropriate follow-up care from the comfort of your home.
Referral Consultation — $75
Need to see a specialist? Our referral consultation includes a provider evaluation to review your symptoms, medical history, and healthcare needs to determine the most appropriate specialist referral. This process helps ensure continuity of care and provides specialists with relevant clinical information to support efficient and effective treatment.
Weight Management Consultation — $150
Begin your weight loss journey with a comprehensive medical evaluation focused on your unique health goals. Your consultation includes a detailed health assessment, laboratory review and ordering when clinically appropriate, evaluation of contributing factors that may impact weight, and a personalized treatment plan. Recommendations may include nutrition counseling, physical activity guidance, lifestyle modification strategies, and medication options when appropriate.
Hormone Optimization Consultation — $150
Our hormone optimization consultation is designed to identify and address symptoms that may be related to hormonal imbalance. The visit includes a comprehensive health assessment, laboratory review and ordering when clinically appropriate, evaluation of symptoms and health history, and development of a personalized treatment plan. Recommendations may include lifestyle modifications, nutritional support, and hormone replacement therapy when appropriate, all guided by ongoing medical oversight.
Peptide Therapy Consultation
A comprehensive peptide therapy consultation is available to evaluate your health history, symptoms, and wellness goals and determine whether peptide therapy may be appropriate for you. The standard consultation fee is $150; however, this fee is waived when peptide therapy is purchased and initiated following your consultation. If treatment is not recommended or you choose not to proceed with therapy, a reduced consultation fee of $75 will apply. Once treatment is prescribed, patients should allow approximately 24–48 hours for pharmacy processing and delivery of peptide medications, although delivery times may vary based on product availability and shipping schedules.
IV Therapy Consultation
A comprehensive IV therapy consultation is available to review your health history, symptoms, wellness goals, and any contraindications to determine the most appropriate IV hydration or wellness infusion for your needs. The standard consultation fee is $150; however, this fee is waived when IV therapy is purchased and initiated following your consultation. If treatment is not recommended or you choose not to proceed with therapy, a reduced consultation fee of $75 will apply. IV therapy pricing varies based on the infusion selected. Patients should allow approximately 24–48 hours for scheduling, preparation, and coordination of IV wellness services.
Follow-Up Visit (Weight Management, Hormone Optimization & Peptide Therapy) — $75
Ongoing follow-up visits are an essential part of ensuring safe, effective treatment and optimizing your results. Follow-up appointments are generally required approximately four weeks after initiating therapy and whenever medication refills, dose adjustments, or treatment plan modifications are needed. These visits allow your provider to assess progress, review any side effects or concerns, monitor treatment effectiveness, and make personalized recommendations to support your continued success.
